Dear Parent or Guardian,
Per the State of Connecticut Department of Public Health Immunization Program, children enrolled in Pre-K are required to receive the Influenza vaccine each school year.
FYI, with the enaction of Public Act 21-6, new religious exemptions can no longer be accepted.
Children who have never received an Influenza vaccine need 2 doses separated by 28 days.
Your child must be in compliance with state regulations or he/she cannot return to the preschool program on January 3, 2023.
Influenza vaccines are available at the Stratford Health Department--468 Birdseye Street, 3rd floor.

If your child has already gotten their flu shot for this school year, please remember to send in the doctor’s form showing immunization, and thank you!
